Kindergarten Enrollment

Important Enrollment Information

Required Documentation

• Parent identification
• Birth certificate for student(s)
• Immunization record for student(s)
• Last report card if applicable
• Current vision screen per Michigan Law
• Health appraisal for all incoming Kindergartners
• Signed Lease, Mortgage agreement, or property tax statement
• Current DTE or Consumers energy bill

Current Webster Early Childhood Center Students:

If your child currently attends Webster Early Childhood Center or participated in the Early 5s Program during the 2019-20 school year, and will be attending Kindergarten, please complete the following Pre-enrollment.

• Complete Pre-Enrollment
• Once completed, please bring the required documentation to the Ford Administration Office. You will then be contacted by a member of our staff to schedule Kindergarten Screening. 

Hazel Park Residents (First Entry Into the District)

If your child is a Hazel Park Resident and did not attend pre-school at Webster Early Childhood Center, click the Complete Pre-Enrollment button to get started.

• Complete Pre-Enrollment
• Once completed, please submit all required documentation to the Ford Administration Office. You will then be contacted by a member of our staff to schedule Kindergarten Screening. 

Schools of Choice (Non-Resident)

• Complete Pre-Enrollment
• Once completed, please print and return the attached Schools of Choice Application to the Ford Administration Office along with all the required documentation. If you are not able to print the application you may complete it at the Ford Administration Office.  

Enrollment Forms

Most of the required forms are embedded into the online enrollment portal. The forms below are additional items that may be needed for enrollment. 

• Health Appraisal (All Students)
• Medication Authorization Form (Only Needed for Student Use of Medication at School)
• Release of Information K-12 (Non-Resident, Schools of Choice)
• Release of Information Alternative Education Schools
